In OR-04, the cardiac surgeon is performing an open-heart aortic valve replacement. To operate safely on the valve, the patient's heart must be completely stopped. You are standing two feet away operating the cardiopulmonary bypass console, which functions as the patient's mechanical heart and lungs.
Maintain physiological blood pressure (MAP 65–85 mmHg) and organ oxygenation (SvO₂ > 70%), prevent catastrophic blood clotting in the machine's plastic circuit (keep ACT > 400 sec), rewarm the patient to 37°C, and successfully wean circulation back to the native heart.

Blood rapidly clots when contacting synthetic plastic tubing. Watch the Activated Clotting Time (ACT). If it dips towards 400s, click 'Administer Heparin Bolus' immediately.
Balance Pump Flow & MAP
Adjust Slider 1 (Arterial Roller Pump Speed) so that Mean Arterial Pressure (MAP) stays in the safe 65–85 mmHg green zone.
Set Oxygen Delivery (FiO₂)
Adjust Slider 2 (Gas Blender FiO₂) around 60%. Monitor Venous Saturation (SvO₂) to verify that vital organs (brain, liver, kidneys) receive enough oxygen.
Rewarm & Wean Bypass
When the surgeon completes the valve suture, click 'Rewarm Patient to 37°C'. Once the heart warms and telemetry is stable, click 'Wean From Bypass' to release the cross-clamp.
